This area contains documents that may be needed by North Raleigh Family Medicine. The documents are provided in two "Electronic Formats"; Microsoft Word format (the "DOC" suffix files) and Adobe Acrobat format (the "PDF" suffix files). If you do not use Microsoft Word, you can download Adobe Acrobat Reader for free (if you do not already have it) by clicking on the link provided below. If you have problems using this Download page, please call our office for help.
Please note that if you already have the Word and/or Acrobat programs installed on your computer, the documents may try to open directly when you click on them. If you just want to download them, simply "right click" on the document link and select "Save Target As..." from the menu, and then select a location on your computer to save the document to.
